The Academic Physician's Guide to RVUs

In academic medicine, the phrase "Publish or Perish" has an equally demanding partner: "Production or Perish." While research and teaching are core missions, clinical revenue often funds the department. Here is how to navigate RVU targets in an academic setting.

The "cFTE" Concept

Your clinical Full-Time Equivalent (cFTE) determines your RVU target. If you are 0.5 cFTE (50% clinical), your target should be 50% of the full-time benchmark.

  • 0.8 - 0.9 cFTE: Minimal protected time; high clinical expectation.
  • 0.5 cFTE: Significant research/admin time; often 2.5 days of clinic/OR.
  • < 0.2 cFTE: Leadership or heavy research focus.

Protecting Your Time

The biggest risk for academic physicians is "mission creep" where clinical duties bleed into research time without RVU credit.

Strategy: Ensure your contract explicitly states your cFTE and that your wRVU target is prorated mathematically. If the benchmark is 6,000 wRVUs and you are 0.5 cFTE, your target is 3,000.
